CEWA MDW Sponsorship Form

If you are not interested in being a Sponsor in Section 1, you can purchase just an ad in Section 2 or be a donor in Section 3.

1) Sponsorship Levels:

Panther Level – $2500

One (1) full page ad
Personalized pop-up banner displayed at HOF Banquet and during at least one dance
Six (6) Scholarship dance tickets
Name listed on printed materials, sponsorship board, and in the program as a sponsor

Warrior Level – $1000

One (1) full page ad
Personalized pop-up banner displayed at HOF Banquet and during at least one dance
Two (2) Scholarship dance tickets
Name listed on printed materials, sponsorship board, and in the program as a sponsor

Blue Level – $500

One (1) full page ad
One (1) Scholarship dance ticket
Name listed on printed materials, sponsorship board, and in the program as a sponsor

2) Ad Sizes:

Full Page (color) – $200

Half Page (color) – $125

3) Donor: Less than $125 (Listed in souvenir jounal as a donor)

With all sponsorship levels, you will receive one (1) full page ad. After completing Section 1, you can upload your ad in Section 4. You do not need to complete Section 2, unless you want to purchase an additional ad(s).

Note: If you selected one of the Sponsorship Levels in Section 1, you may skip this Section 2 because you will receive one (1) full page. Go to Section 4 and upload your ad. If you want to purchase an additional ad, please complete this Section 2 and upload your ads in Section 4.
