CEWA MDW Sponsorship, Ad and Donation Form

If you are not interested in being a Sponsor, you can purchase an ad or be a donor.


1) Sponsor Levels:

Panther Level – $2500

  • One (1) full page ad
  • Personalized pop-up banner displayed at the HOF Banquet and during at least one dance
  • Six (6) Scholarship dance tickets
  • Name listed on printed materials, sponsorship board, and in the program as a sponsor

Warrior Level – $1000

  • One (1) full page ad
  • Personalized pop-up banner displayed at the HOF Banquet and during at least one dance
  • Two (2) Scholarship dance tickets
  • Name listed on printed materials, sponsorship board, and in the program as a sponsor

Blue Level – $500

  • One (1) full page ad
  • One (1) Scholarship dance ticket


2) Ad Sizes:

Full page (color) – $200

Half page (color) – $125


3) Donor: Less than $125 (Listed in souvenir journal as a donor)
